  • How do I add lives on my policy?

    Fill a Donewell funeral endorsement form.
    Client must indicate the name and date of birth of the new life E.g. father, mother, spouse, etc.
    Client must also indicate the current state of health on the form.

  • How do I remove a life covered on the policy?

    Fill a Donewell endorsement form or submit a letter to remove life. Client must indicate the name of the life he wishes to remove from the policy.

  • How do I modify the number of beneficiaries?

    Fill a Donewell endorsement form or submit a letter to add or delete beneficiary to policy.
    Client must indicate the name and date of birth of the new beneficiary, as well as the relationship of the beneficiary to the client.
    Client must also indicate in percentage, the distribution of benefit payment at death.

  • How do I modify method of payment?

    To other Pay points
    Client must fill and sign Donewell mandate form.

    To Controller
    Client must fill and sign the controller mandate form.

  • How can I change my address?

    Fill a Donewell endorsement form and indicate the new address and contact details or submit a letter indicating the new address and contact details.

  • How can I change my trustee?

    Fill a Donewell endorsement form or submit a letter to change trustee details.
    Client must indicate the name and date of birth of the new trustee, the relationship of the trustee to the client, as well as the contact details of the new trustee.

  • How do I make a claim?

    By completing a claim form and submitting it to any of our offices.
    For surrenders and maturities add policy document and a nationally recognized photo ID card.
    For loans, withdrawals, refunds and reimbursement add only nationally recognized photo ID card.

    The following documents must be submitted in the case of death claim:

    1. Death certificate
    2. Medical certificate of cause of death
    3. Police report, in the case of accidental death
    4. Policy Document
    5. Nationally recognized photo ID
  • How do I change the name on my policy?

    Client must provide proof that new name and the name on the policy refer to the same person. Proof may be in the form of:

    1. An affidavit or marriage certificate
    2. A gazette
