    Requirements

    1. A completed DEATH claim form
    2. DEATH/BURIAL/Medical Certificate/Doctor’s Report
    3. Proof of Identity of Beneficiaries/ Trustee
    4. Policy Document
    5. ID card of deceased.

    Requirements

    1. A REFUND form (accurately completed and signed by the client)
    2. A signed copy of client’s id (nationally recognized ID).

    Requirements

    1. Clients Policy Document (An Affidavit signed by the Client in the case of a Missing Document)
    2. A SURRENDER Form (Accurately Completed and Signed By the Client)
    3. A Signed Copy Of Clients ID (Nationally Recognized ID).
    4. Susu Card of the Client (In The Case Of Edwadifuo or Skuul Boafo Surrender)
